POSITION PROFILE #JOS000726A: *** JOB TITLE: SLP / Speech-Language Pathologist / CCC or CFY (EI) *** JOB TYPE: Temporary OR Permanent Employee Full time OR part time. 10 OR 12 month position. *** SHIFT/HOURS/CALL/ROTATION: Monday through Friday 8am to 3pm and up to 4pm as needed. 6.5 hour work days / 32.5 hour work weeks. Approx. 1 690 hours per year worked. *** PAY SCALE: $40k-CFY CCC-42K+ BOE. PAID PER SESSION. *** RELOCATION ALLOWANCE / SIGN-ON BONUS: Available for Permanent Hire only. *** CERTIFICATIONS/REGISTRATIONS/LICENSE/EDUCATION REQUIRED: Master degree in SLP. NYS license. *** APPROXIMATE STARTING DATE: 2/18/08 (ASAP) *** DESCRIPTION of FACILITY/PRACTICE & LOCATION: Free-standing regional developmental evaluation center dedicated to EI pediatrics. 50% on site 50% in-home cases within 15 mile radius of facility. Infant preschool toddler daycare a special ABA (Applied Behavioral Analysis) classroom. Team approach and a great mentoring program in place to complete all goals for each CFY. Upstate NYS. 65 miles of a metro area. Near international airport with direct jet service to most major cities in the US. A reasonable distance from Canada the St. Lawrence River & the Adirondack Mountains. Large selection of year round sports. High quality education (each student is required to complete the ''NYS Regents Exam'' which is required of all schools). There is a large selection of private schools which are inexpensively priced. The region has rolling hills and many lakes. Many colleges and large universities with division one ranked football basketball & lacrosse teams. Moderate summer climate & seasonal climate changes that are beautiful. *** STAFF/TEAM (Description): Will work predominantly autonomously staff includes 3 SLP’s 2 CFY’s 2 PT’s 2 OT’s 1 TSHH 2 SPED 3 REG.ED 6 RN’s 1 LPN’s 15 Teacher Aides 6 Certified Teaching Assistants. *** JOB DESCRIPTION: SLP will work with Early Intervention only. No school-aged children. 80% home based 20% clinic. Maximum mileage per week is 250 miles reimbursed at .30 cents per mile. Daily average is 50 miles. SLP will see 5 clients per day. Will work with one other SLP. Both SLPs split the county. * Conduct evaluations on children from birth to 5 years old. * Diagnose and evaluate speech and language skills as related to educational medical social and psychological factors. * Development & implementation of goals plans and objectives for children from birth to 5 years old. * Plan direct and conduct rehabilitative treatment programs to restore communicative efficiency of individuals with communication problems of organic and non-organic etiology. * Conduct annual evaluations of children from birth to 5 years old. * Assess children needs for augmentation and adaptive equipment and help in securing necessary equipment. * The SLP must be familiar with ***** Dysphagia Cognitive Impairment and Neurogenic communication. * Provide counseling and guidance and language development therapy to disabled children. * Review individual file to obtain background information prior to evaluation. * Develop and implement individualized plan for assigned patient to meet individual needs interests and abilities. * Review treatment plan assess individual performance. Modify change or write new programs. * Maintain records as required by law established policies and administrative regulations. * Participate in if sp / individual family service plan & meetings. * Attend meetings with the committee on pre-school special education. * Complete all required documentation including registers and discharge summaries. * Complete incident reports and documentation of minor injuries. *** BENEFITS: There is NO PAID TIME OFF for the first 30 days of employment.
