Job Description
SUMMARY OF MAJOR FUNCTIONS: This position provides skilled assessment and physical therapy treatment services to patients. The PT directs and guides patient activity as related to physical therapy. This position works collaboratively with staff Medical Staff Administrator Support Services Hospice Volunteer Services and Ancillary Services to provide quality patient care. REQUIREMENTS / QUALIFICATIONS: Eligible to be licensed to practice physical therapy in Maine. BCLS. Upholds the APTA Code of Ethics Previous home health experience preferred. Reliable vehicle with current automobile insurance. Obtains and maintains hospice certification per state and federal regulations prior to caring for hospice patients. ESSENTIAL FUNCTIONS: Models the organizational values (BEST) through open positive communication team work respect for coworkers assertive problem solving sound decision making and a commitment to quality for patients and others. Provides a clean safe and secure environment for patients and others by following established safety policies and procedures in the home and at the office. Provides appropriate age specific care to infant pediatric adolescent adult and elderly patients. Maintains competency through ongoing training and education including mandatory inservices yearly. Provides instruction as needed for staff regarding physical therapy techniques and safety measures. Performs patient evaluations recommends treatment procedures modalities gait and exercise programs and implements it. Develops Plans of Care and keeps recertification up-to-date. Interacts with physicians providing patient evaluations reports of patient status in physical therapy and recommendation for further or additional treatment of service. Documents patient’s progress in medical record. Advises nurses and home health aides regarding patient’s physical therapy program. Interacts with patient families and others involved with patient care. Supervises and instructs physical therapy assistant in specific patient procedures. Collaborates with staff regarding patient’s progress to insure consistent treatment. Demonstrates competency in the use of required equipment and procedures for patient care.
