EXCEPTIONAL CHILDREN DESERVE EXCEPTIONAL THERAPISTS.... Physical Therapist Area: Conroe TX This hospital based home health agency of N. Houston in Conroe TX is currently seeking a Physical Therapist. Duties of this position inlcude the following: Initiate patient contact on referral. Demonstrate ability to provide patient screening and make appropriate recommendations. Select and administer the appropriate evaluation and consult with others regarding interpretation. Treatment planning is comprehensive realistic and involves the patient and/or family. Demonstrates ability to prioritize treatment. Implement and adjust therapeutic appropriate programs with specific individualized treatments for an assigned caseload. Complete appropriate dicsharge planning in a timely manner. Attend and participate in facility and department meetings. Communicate effectively with staff patients family and others. Present oral reports in professional form content and manner. Follows policies and procedures. Current TX PT License. Current CPR. This position requires a graduate from an accredited school of PhysicalTherapy. Recent graduates who have applied for the registration exam may be considered for provisional appointment.
