POSITION PROFILE #JOS000000939: *** JOB TITLE: PTR / Outpatient Orthopedic / Physical Therapist *** JOB TYPE: Temporary to Permanent/Retained Employee Permanent/Retained Employee Full-time. *** SHIFT/HOURS/CALL/ROTATION: Monday through Friday 8:00 am to 4:30 pm. *** PAY SCALE: $25-30 per hour BOE. *** CERTIFICATIONS/REGISTRATIONS/LICENSE/EDUCATION REQUIRED: New Grads are welcome to apply. NY PT license or applied. *** APPROXIMATE STARTING DATE: 2/25/08 (ASAP) *** DESCRIPTION of FACILITY/PRACTICE & LOCATION: Outpatient orthopedic practice. Two (2) offices in Syracuse NY provide coverage as needed to both. *** STAFF/TEAM (Description): Other PT’s on staff will work autonomously *** JOB DESCRIPTION: * Review Physicians referral (prescription) and patients condition and medical records to determine physical therapy treatment required. * Test and measure patients strength motor development sensory perception functional capacity and respiratory and circulatory efficiency and records findings to develop or revise treatment programs. * Plan and prepare written treatment program based on evaluation of patient data. * Administer manual exercises to improve and maintain functions. * Instruct motivate and assist patient to perform various physical activities and in use of assistant and supportive devices such as crutches canes and prostheses. * Administer treatments involving application of physical agents. * Evaluate effects of treatment at various stages and adjusts treatments to achieve maximum benefit. * Administers massage and traction to relieve pain. * Record treatment response and progress in patients chart or enters information into computer. * Instruct patient and family in treatment procedures to be continued at home. * Evaluate fit and adjust prosthetic and orthotic devices and recommend modification to Orthotist. * Confer with Physician and other practitioners to obtain additional patient information suggest revisions in treatment program and integrate physical therapy treatment with other aspects of patients health care. * Orient instruct and direct work activities of assistants aides and students. * Plan and conduct lectures and training programs on physical therapy and related topics for medical staff students and community groups. *** BENEFITS: * VACATION TIME: Earn 1 day per month. Accrue up to 10 days the first. Can use 3 days after 90 days of continuous employment. * SICK / PERSONAL TIME: After 90 days up to 40 hours of documented time. * PAID HOLIDAYS: After 90 days of employment: 8 major holidays paid. * HEALTH INSURANCE: After 90 days of employment. Single $57.91 bi weekly contribution. Employee and Spouse contribution $661.35 Employee and child $481.84. Family $875.81. Contribution by employer is $185.00 per month. United Health CPA EPO.
