Case management responsibilities for individuals with developmental disabilities, physical/illness or mental illness included coordinating services to enable individuals to receive a full range of appropriate services in a planned, coordinated, efficient and effective manner to promote the well-being of the individual. Additional responsibilities included locating, managing, coordinating, and monitoring: a) all proposed services; b) needed medical, social, educational and other publicly funded services, regardless of funding source; and c) informal community supports needed by eligible persons. Developing a long term and short-term plan of services, based upon the needs and interests of the individual, and developing the financial support plan that was needed to support the services requested. Planning and advocating for the individual, linking the individual/family with available community services, monitoring the individualís satisfaction, as well as monitoring all services delivered to the individual were designed to assist individual. Completed Person Center Planning assessments for individuals/families. Completed comprehensive environmental and Psychosocial assessments to identify clients needs.

Assessed clients physical and mental health status including problems faced by the individual and their family through a comprehensive Psychosocial, PCP, and environmental assessments. Recommendations were made regarding a treatment plan, educational information, referrals to community resources, and other supportive services.
