Obtains a complete physical/mental health history as a basis for formulating assessment and treatment goals.
Gathers information regarding the client’s physical health through observations, personal histories, and other assessment measurements.
Consults with experienced nurses regarding patient care plans and/or interventions.
Collects all appropriate clinical and laboratory data, reporting any abnormalities to appropriate provider[s].
Develops a comprehensive treatment plan based on nursing assessment and standards of care.
Implements appropriate and most effective therapies for identified condition(s).
Monitors patient’s progress and regularly reassesses patient’s condition, recommending appropriate adjustments to treatment plan.
Other duties as assigned.

Providing patient care, assessments, and completing MD orders on the Geriatric and Hospice patients.

Monitor vital signs and performing appropriate interventions and protocols.

Wound care assessment, intervention including wound treatments.

Administer medication and treatments, including documentation of interventions.

Acquired cross-functional training in acute care, operating room, medical ambulatory, pediatric, post surgical & wound care, OB, medical and surgical floor, ICU.

Assisted with direct of patient care and self-management education in a pre/post-op environment.

Observe patients, charting, & reporting changes in patients' conditions, such as adverse reactions to medication or treatment, and taking any necessary action.

Provide standard care and treatments, such as assessing and documenting vital signs, wound care, treating bedsores, massaging, and performing catheterizations.
